Public Health Nurses provide care coordination services to promote the best possible health for pregnant women and their babies in the New River Health District. If you are pregnant or have a child less than two years of age, you may be eligible!
Known as the BabyCare Program, care coordination services are covered by Medicaid, FAMIS, and the VaPremier Health Plan. Serving as a Care Coordinator, the public health nurse (PHN) is a registered nurse who works with the client and physician to identify and address problems or needs that can impact the well being of the mom, unborn baby, or infant. Together, they develop and agree upon a written plan that addresses the client’s unique needs and supports the physician’s directives.
It is important for the client and PHN to maintain regular and frequent contacts in the home, clinic, community, or by phone. The PHN offers current health information on pregnancy, childbirth, and other women’s health issues. She helps the mom or prospective mom prepare for her new baby and/or small child by teaching her about normal growth and development, nurturing and feeding an infant, keeping regular appointments with the doctor, recommending childhood immunizations, locating and coordinating other community resources, and addressing other issues faced by parents. The PHN is a consistent and reliable professional who supports the client in managing future appointments and responsibilities that accompany pregnancy and parenting.
“Being pregnant was exciting, but it was also frightening,” states Kasie Riffe, a BabyCare Program participant. “My public health nurse has been with me through the whole experience. She helped me to be prepared for my baby. She teaches me what is normal, and also when I should call my doctor. My baby is healthy and happy!”
