Doctors Connected hopes partnership with Medicare will help focus patient care

ROANOKE -

At Carilion Clinic doctors and nurses already coordinate patient care, use electronic services for prescriptions, and nurses take technology with them to every patient room.  Now, through Doctors Connected, Carilion is working together with Medicare to get a better idea of what is missing for patients in our communities.

Doctor Stephen Morgan is Carilion's Chief Medical Information Officer.

He says, "That information is used to essentially see what patients are doing in our system and outside our system from a claims perspective. Claims data from Medicare."

Carilion Clinic started the Doctors Connected Program in January of this year.  They have about forty six thousand patients they will be able to collect data from.  It will take about three years for the entire program to identify any gaps in care or trends in medical care, but they are hoping for some results in April

Dr. Morgan says, "From a providers perspective we would like to do the care coordination and collaboration with all of our patients.  That is how it will benefit the community."

Right now the Center for Medicare and Medicaid Services has two hundred twenty Accountable Care Organizations in the U.S. Virginia has 7 ACOs and Doctors Connected through Carilion Clinic is one of them.

There is a financial incentive for Carilion Clinic doctors to prevent illnesses that lead to what can be expensive trips to the hospital.  If this program is successful the data has room to grow.

Dr. Morgan explains, "As a system we have taken the approach that we will apply this to all of our patients eventually."

With the goal of better care and less cost to the community.
